Risk Form

All participants must sign an acknowledgment of risk form before rafting with A Wanderlust Adventure. Participants under the age of 18 must have a parent or legal guardian sign the form. An online waiver will be sent to you in your email confirmation. There will also be a registration form that needs to be filled out for each participant. The registration form will ask for your address and an emergency contact back home. Any participant under the age of 18 without their parents signature on this form will not be able to raft. We recommend that everyone take care of their online waiver right after you book your reservation. It will hold things up at check in if you have to take care of your waiver then. All waivers should be filled out before arriving to our meeting area if possible. Only parents are allowed to sign this form for their children. Thank you!
